Qualifications

 

5.1.1.1. Education. Master of Social Work (MSW) degree from an accredited graduate school of social work or a Bachelor of Science in Nursing (BSN) degree from an accredited program.

 

 

5.1.1.2. Experience. A minimum of two years full-time experience in care coordination, discharge planning or case management in a medical setting within the last (5) years.

 

5.1.1.3. License/certification. Social worker shall have current, active, valid and unrestricted Social Work License from any states of U.S.; or authorizing document from any U.S. state, District of Columbia; or U.S. territory or jurisdiction to provide health care within the scope of practice for a specific health care discipline.

 

 

5.1.1.4. Basic Life Support (BLS) Certification. Contract SNCs shall maintain current certification in Basic Life Support (BLS) from the American Heart Association, or the American Red Cross. Certification cards must display the American Heart Association,

the American Red Cross or other approved certifying body emblem. Web-based (on-line) classes are not acceptable by the Government. The Contractor shall not bill the Government for contractor time to attend Basic Life Support (BLS) certification training.

 

 

- 자격조건 -

1. 공인 사회복지대학원 석사학위(MSW) 또는 공인 프로그램의 간호학 학사학위(BSN)

2. 최근 5년 이내 2년이상의 의료 환경에서 돌봄 조정, 퇴원 계획등 사례관리 분야에서 정규직 경력

3. 미국 사회복지사 면허증 (미국 내 모든 주에서 발급한 유효하고 제한 없는 사회복지사 면허)

4. BLS

1. 5년 계약직 (재입찰후 연장가능)

2. 연봉 9천만원 (조정가능)
